Clinically checked · Our review process

Key points

  • DirectMeds charges one flat $147/month for semaglutide or tirzepatide at every dose. That is unusual: across our ranking, tirzepatide usually costs $40-$110 more than semaglutide.
  • The medical process is sound. A licensed physician reviews your intake and can decline you, and there is no membership fee, consultation charge or insurance requirement. Shipping is free in 1-2 days and you can cancel anytime.
  • DirectMeds' needle-free sublingual drops are a legitimate option prescribed at clinician discretion, but the major trials studied injections, which remain the standard of care. Know that trade before you choose drops.
  • For tirzepatide, $147 is one of the best prices available (only welludr at $89 and embody at $119 are lower). For semaglutide it is mid-range, beaten by welludr $49, embody $69, altRx $89 and HealthRx $99.

Is DirectMeds a legitimate GLP-1 provider?

Yes. DirectMeds requires a licensed physician review before prescribing, publishes one flat price, and adds no hidden fees. Two of its claims stand out from the rest of the market, though, and both are worth checking rather than taking on trust:

  1. Pricing: $147/month flat for either semaglutide or tirzepatide, at every dose.
  2. Format: you can take your medication as weekly injections or as needle-free sublingual drops.

The first is an unusual business model. The second is an unusual clinical choice. We look at each below.

In speaking to your reps I found them very polite and helpful

“In speaking to your reps I found them very polite and helpful. I appreciated all the help. However, the doc was not very helpful. I couldn't access his message. However, I did get a message that my meds were on the way. Since I am not good at accessing my account, it is probably my fault. I would prefer to speak to your reps rather than go through an on line account.”

My first bottle of NAD that I received

“My first bottle of NAD that I received was 50 mg. I was to take 20 units Monday through Friday. I only had enough medication for two weeks. I then called and requested an increase to 1000 mg of NAD and I am waiting for it to be delivered. I want to be sure that I will receive enough for 30 days. My first batch was only enough supply for two weeks and I was sent no syringes. Are you going to make up for the shortage?”

I had an order placed since Sep 4th

“I had an order placed since Sep 4th. It did not actually ship until I had called 3 times. It just said "Label Created". The order was damaged in transit and the contents discarded. When I went online to the chat, the AI kept saying the order was delivered. I had to call in and get things straightened out. Now I am waiting for the replacement order to ship.”

Is DirectMeds' $147 really the same at every dose?

Yes, and that is rare. Everywhere else in our ranking, tirzepatide carries a $40-$110 premium over semaglutide. DirectMeds charges the same for both.

Flat pricing by dose also removes a common industry habit: advertising the starter-dose price and raising it as you titrate up. Neither practice suggests cutting corners. Both point to a company competing on price transparency.

Does DirectMeds charge anything beyond $147?

No. Here is what sits around the monthly price:

  • Doctor review: a licensed physician reviews your health intake before anything is prescribed and can decline treatment.
  • No membership fee and no consultation charge.
  • No insurance requirement.
  • Free shipping in 1-2 days.
  • Cancel anytime.

The monthly price is the whole relationship. In a market where the classic complaint is a surprise charge, having this little fine print is itself a trust signal.

DirectMeds GLP-1 plans and prices

Every plan DirectMeds publishes, with its terms listed under each price.

Are DirectMeds' needle-free GLP-1 drops as proven as injections?

No, and DirectMeds patients should know that going in. The major clinical trials behind semaglutide and tirzepatide tested injections, and weekly injection is still the standard of care.

Compounded sublingual versions are prescribed at the clinician's discretion, dosed differently, and give up some evidence in exchange for easier use. That is a legitimate clinical option, not a scandal. Its best use case is clear: if needles are the only thing stopping you from starting treatment, drops can turn "never" into "started," and your prescribing doctor makes that decision with you.

Is GLP-1 Treatment Right for You?

GLP-1 medication is prescription-only: a licensed clinician has to decide it is appropriate, and it is not a fit for everyone. Side effects are possible, and they tend to show up in the first few weeks while your body adapts.

Results are usually best when the medication is paired with simple daily habits, such as balanced meals and regular movement.

For both safety and lasting progress, pick a provider that screens you properly before prescribing and keeps following up once treatment starts.

Where does DirectMeds lose on price?

DirectMeds is a bargain for one medication and average for the other:

DirectMeds also keeps its model lean on purpose: no coaching, no dietician, no brand-name medications. It wins specific comparisons, not every one.

Who is DirectMeds the right choice for?

DirectMeds passes our checks: physician-gated prescribing, transparent flat pricing, straightforward shipping and no fee games. It fits two people especially well:

  • Tirzepatide patients, for whom $147 flat at every dose is among the best deals available.
  • Needle-averse first-timers, for whom the drops option may be the difference between starting treatment and not starting at all.

Anyone taking semaglutide by injection should compare the cheaper semaglutide providers before signing up.
